Enrollment Checklist
 I have verified my child's school placement. School assignment is made by my address. To verify school placement, I should contact the IRCSD Attendance Office at (772) 564-3145. Districts determined by maps are available at public libraries and at each school site.

I have determined where I must go to enroll my child. Students in high school go to the school site to enroll. (Districts determined by maps available at public libraries and at each school site.)

I will take a copy of my child's Birth Certificate with me when I enroll him/her.

I will take my child's Social Security Card with me when I enroll him/her.

I will take Acceptable Proof of Residency with me verifying my CURRENT address and also ensuring that my child is enrolled in the correct school for the zone in which I reside when I enroll him/her. A driver's license is NOT acceptable proof of address. If I have moved and bought a home, I can bring a bill of sale or deed to the home with the current address If I have moved and am renting, I can bring a rent receipt that has the current address and an electric bill, cable bill, or telephone bill in my name at that address.
If I have moved and am temporarily staying with someone, two requirements must be met:
A notarized statement from the person who owns/rents the home verifying that I and my child are living at that address Evidence that I am receiving mail at this address with the child/children

I have my child's Health Records (Immunizations/Physicals) ready to present to the school at the beginning of the school year; The attendance office does NOT handle health related matters, including Proof of Physical and Certificate of Immunization.

Remember, the attendance office does not handle health related matters, including Proof of Physical and Certificate of Immunization. These must be presented to the particular school my child is attending at the beginning of the school year. (Blue card for shot records may be secured from health department of individual physician's office.)

I have my child's withdrawal grades and transcripts form the previous school he/she was attending.
